We want to make sure you enjoy a smooth and stress-free visit with us. Please arrive at our office at least 15 minutes early to park and register with the receptionist. Don't forget to bring your parking ticket with you and we will validate it. In preparation for your first visit to FPMU, please fill out and bring the Patient Information Form and Patient Medical History Form, which was mailed to you when your appointment was made. You can also download forms below. Your medical records are very helpful to the physician, and as such are essential on your first visit. Please fill out the Authorization to Release Medical Information Form. You can also find a copy of GRWH's Privacy Practices below. If any lab work is needed, please be prepared to notify us if your insurance company has special laboratory preferences to receive the highest benefit available for these services. |
