Office Policies & Insurance

Children under the age of 18 must have a parent with them during each office visit.

Payment is due at the time of service unless other arrangements are approved in advance. If you have co-pay insurance, please be prepared to pay at the time of your visit. For your convenience, we accept Visa, Mastercard and Discover. Should you have any questions regarding our fees or your insurance coverage, please call the billing department in advance of your appointment. Please have your insurance information available at each visit.

Please make sure to bring your most recent insurance card along with any paperwork sent to you. If no insurance card is present you will be responsible for payment at time of service.
